A	FORM 6 AND/OR 7-SOLO-ENS AND PERCUSSION ADJUDICATION SHEETS FORM 8-MEDIUM ENSEMBLE ADJUDICATION SHEETS. These forms are prepared via the “ONLINE ENTRY” procedure and will be printed by the executive secretary. You can access “ONLINE ENTRY” from front page of Region 12 website or the link above. Directors should “log-in” and then click on the proper form from the top of the page and complete the entry process.
B	MAKE ONE COPY OF THE "REVIEW ENTRIES" PAGE (formerly called a FORM 9) FOR YOUR RECORDS AND HAVE EACH STUDENT INITIAL THEIR ENTRY SO THAT YOU HAVE A RECORD OF HAVING THEM ENTER.

D	WHAT TO BRING THE DAY OF THE CONTEST
	Completed FORM 1 (buff paper) listing all eligible competing students. List alphabetically last name, first name, grade in school [Ex: Jones, Jonathan 09], signed by the principal or superintendent. You may use the Form 1 (Cover Sheet) that is posted on the frontpage of our region website or a Form 1 prepared via the ONLINE entry. Attach a class roster to the form.
