MAIL (if you wish to do so) ONE COPY FORM 1 (signed by the principal) - List the entire "eligible" membership of the competing parent organization in alphabetical order. This form must be in the hands of the CONTEST CHAIRMAN prior to your students' performance. It is not necessary to mail this form with your other forms and fees. |

(If your school business office is unable to prepare
a check to be mailed "by the deadline", MAKE A COPY
of the Purchase Order (or Check Request) and MAIL
IT with a copy of the Computation Form 1A This provides "confirmation" that the check request was made in a timely fashion prior to the 30-day deadline and meets the UIL C&CR requirements that entries "must be postmarked" or "in-the-hands of the contest chairman" 30-days prior to the first day of the event. |

MAIL ONE COPY REVIEW ENTRIES REPORT "REVIEW ENTRIES REPORT" is available after you have completed your entries via the "online entry". You are required to mail a copy of the Review Entries pages with your other forms and fees. The REVIEW ENTRIES report must be signed by the students [each soloist; only one member of an ensemble needs to sign] and signed by the director verifying students name, PML ID#, title of selection and composer, and the total events entered for the respective Solo/Ensemble contest. |

F. "Late Fee" for solo/ensemble contests
"Review Entries"report (formerly called a Form 9), signed by students AND
director, must be postmarked 30-days prior to the first day of the contest WITH COMPUTATION FORM
1A and CHECK for FEES ATTACHED.
Changes to titles of selections may be made up to 7-days prior to the event and the changes are made by the director
via email through the Region 2 UIL Music Office. {Forms are all printed by the UIL Music Office and are normally
printed 7 days prior to the contest.)
Any entry submitted after the 30-day entry deadline (up to the day of the contest) is considered a NEW ENTRY
and subject to the LATE ENTRY schedule of fees. A form "LATE SOLO/ENSEMBLE ENTRY FORM" is available on
the Region 2 UIL Music website.
FEE for "late entry" will be $8.50 normal fee PLUS $25 per soloist and $15 per ensemble member
The FEES DUE for "new" or "late" entries must be paid PRIOR to scheduled performance. "NO
PAY…..NO PLAY!"
