	FORMS 6 - 7 - 8 are prepared via the "ONLINE ENTRY" procedure and will be printed by the executive secretary.
B	WHAT TO SEND: SOLO & ENSEMBLE ENTRY
1.	SEND ONE COPY FORM 1 - List the "eligible" students in alphabetical order. This form must be in the hands of the CONTEST CHAIRMAN prior to your students' performance.
	It is NOT necessary to send a FORM 9 as the information is retrieved from the ONLINE entry by the executive secretary "or" contest chair.
2.	SEND ONE COPY "COMPUTATION FORM for FEES with CHECK (click here)
3.	SEND ONE CHECK for FEES
	PERSONAL CHECKS WILL NOT BE ACCEPTED FOR ANY FEES
